Please print these forms and return with the copy of your payment directly to the Mazurkas Travel Congress Bureau by May 30, 2002.:

Cancellation: The fee will be refunded in 80% less bank charges for cancelation received by Mazurkas Travel before June 30, 2002. No refunds can be made after June 30, 2002.
